This can't be explained in few steps how it can be done but what i can tell you is that you need to have good photographer with great understanding of light and all photography related things and person who is good retoucher.

Overall portrait retouching can't be explained in few steps because it involves a lot of things and great knowledge of light, reality, Photoshop etc...

I can recommend you really great tutorial for retouching. Is from Lynda.com - Photoshop retouching techniques with Cris Orwig. It is far the best i have seen on that retouching subject on the market and i watched quite a lot of them.
